Vitamin B3 for Rosacea?
Bob says, " i began taking vitamin B3 (niacin) 500mg twice a day to lower my cholesterol, and my rosacea is gone. I have been clear for three weeks now, even when eating foods that have caused flare ups....go figure! No p&p's at all! Flushing Caused By Beta-arrestins?
A recent article on the NRS web site with the title, Flushing Cause Uncovered, states:
"Researchers have now identified the molecular pathway for flushing caused by niacin -- also known as vitamin B3 or nicotinic acid, and found in many foods....suggesting that it is instead the beta-arrestins that cause the flushing.... 'By defining the molecular pathway for flushing that begins with niacin, new therapies might be developed to block this process," Dr. Walters said. He noted that other causes of flushing may utilize the same pathway, suggesting it could also be an important mechanism for flushing in general.' "
Whether these 'other causes' of flushing are known, many rosaceans who are interested in avoiding flushing will not doubt be following the research that may develope due to this study by Dr. Walters.

A report indicates that antibiotic "resistance to doxycycline can occur in a few days, with resistant bacteria in abundance after just seven days of treatment with 100 mg doxycyline daily according to a presentation at the Skin Disease Education Foundation Hawaii Dermatology Seminar. Oxymetazoline has been reported in two cases to help rosacea which was reported in the Archives of Dermatology, November, 2007 according to pubMed.

Necessary to perform individual bacterial cultures and antibiograms in rosaceans?
A new study on acne and rosacea patients concluded these findings:
CONCLUSIONS:
1. In the cases of acne vulgaris the majority of isolated bacteria from conjunctival sac included Streptococcus spp., Staphylococcus spp. and Enterobacteriaceae.
2. In the severe cases of rosacea the main bacteria found in conjunctival sac were S. aureus, S.pyogenes, P.aeruginosa, E. faecalis, A. baumanii, P. fluorescens.
3. Because of changeable drug-sensitivity of bacterial strains, it seems to be necessary to perform individual culture and antibiogram in every patient with inflammatory lesions, in particular in clinically severe and resistant to therapy cases of acne vulgaris and rosacea.
4. The higher frequency of the bacterial colonisations in the conjunctival sac in patients with acne vulgaris and rosacea can be a potential source of ocular infections in the cases of local and systemic disorders of protective mechanisms.
5. Estimation of bacterial flora and antibiotic sensitivity of bacteria isolated from conjunctival sac, the skin of the eyelids and skin lesions should be perform, especially when patients are prepared for eye surgery. more info

A new study, "Psychosocial impact of rosacea," says "It was demonstrated that in comparison to the control group, patients with rosacea are less satisfied with their lives, subjectively feel that they receive poor social support, develop great intensity of symptoms, have difficulty in functioning in everyday life, worse general health perception, along with a higher level of anxiety and depression." more info
Rosaceans Less Likely to Use Alternative Treatments for Rosacea?
The authors of a report say that "rosacea sufferers are much less likely to use complementary and alternative medicine compared to sufferers of psoriasis and other dermatological conditions" according to one source. If this were true then there surely wouldn't be hundreds of alternative treatments for rosacea and hundreds, if not thousands of rosaceans discussing alternative treatments for rosacea in various rosacea discussion groups and how they have tried conventional treatments offered by physicians that were wanting. Severe exacerbation of rosacea induced by cinnamon supplements
"The authors report a case of a 68-year-old Caucasian female with type 2 diabetes mellitus who experienced an acute exacerbation of her rosacea 2 weeks after self-initiating cinnamon oil pills to lower her blood sugar levels. Historically, cinnamon oil has been used for a variety of medicinal purposes, but recently the use of cinnamon oil in lowering blood glucose and cholesterol levels in patients with type 2 diabetes is being investigated and gaining popularity amongst the general population. The use of cinnamon has commonly produced cutaneous side effects of irritant or allergic contact dermatitis and been reported to have vasodilatory effects. Yet, there are no reports of cinnamon use triggering a rosacea exacerbation in the literature." source
Skin Test for Demodex
Another clinical study has revealed demodex as a factor to consider in evaluating patients and concluded:
"Demodicidosis should be included in the differential diagnosis of facial eruptions in patients receiving phototherapy." source >

The good thing about this report is that it concludes what I have been saying that a differential diagnosis should be done by taking a skin test for demodex. Physicians don't do this. The assume that demodex plays an insignificant or minor role in rosacea and they don't perform this test.

"...she was ordered Aug. 6, 1878, a mixture containing sulphate of iron and sulphate of magnesia, and for local application an ointment of sulphur, four parts, cosmoline, ninety-two parts. This was followed by rapid improvement, and when seen again on Sept. 17, all trace of the eruption had disappeared, and she felt much stronger and better." source

According to the Washington Post, Aframomum melegueta, a member of the ginger family may have some powerful anti-inflammatory properties. Ilya Raskin, a biochemist at Rutgers University's Biotechnology Center has developed '006' which is derivative compound that works in a similar way to the well-known anti-inflammatory drugs Vioxx, Celebrex and Bextra but, it is hoped, without their side effects, said Raskin and other scientists.
Raskin first became interested in Aframomum during an international effort to search for medicines from plants. "Aframomum contains compounds called gingerols, which are chemically similar to other anti-inflammatory compounds," he said. "That's what initially drew our attention to the plant, and was confirmed in the lab."
Ilic is a visiting scientist at Rutgers, which has patents pending on Aframomum-related discoveries. Rutgers has licensed those rights to Phytomedics, Raskin said.
Phytomedics has licensed cosmetic-only rights to Avon Products Inc. to manufacture skin-care products that contain Aframomum, said Tolo Fridlender, president of Phytomedics.
Avon scientist Xiaochun Luo said the cosmetic's development is based on what Luo calls Aframomum's superior ability to counteract skin irritation. Avon expects to release the products next spring or summer. source

New Acne & Rosacea Society Formed

Society has been formed.
A dedicated, varied group of dermatologists met recently at the American Academy of Dermatology annual meeting with a singular focus: elevating the understanding and treatment of acne and rosacea. Gathering for the first time, members of the newly established American Acne & Rosacea Society (AARS) met to discuss the group's goals and objectives and to hear an entertaining keynote address from renowned dermatologist Albert Kligman, M.D., Ph.D.
According to AARS President Guy Webster, M.D., Ph.D., "AARS was formed to promote research and education on the biology and treatment of acne. Although acne and rosacea are common diseases that make up a large part of daily dermatologic practice, the academic community pays little attention to them. "We aim to correct this imbalance by encouraging investigation and developing young investigators. Educational programs will be created to aid in resident training, and our meetings will provide a forum for presenting the latest research." In addition to Dr. Webster, other Executive Committee members include President-Elect Hilary Baldwin, M.D., and Lee Zane, M.D., who is s Secretary-Treasurer. Niacinamide-containing facial moisturizer improves skin barrier and benefits subjects with rosacea.
A growing body of literature suggests that some moisturizers can improve stratum corneum barrier function, as well as ameliorate dry skin. The clinical signs and symptoms of rosacea, which include increased facial skin dryness and sensitivity, suggest a possible role for such moisturizers as an adjuvant in the management of this condition. This randomized, investigator-blind, controlled observational study (N = 50) was designed to assess whether a niacinamide-containing facial moisturizer would improve the stratum corneum barrier and thus provide a clinical benefit to subjects with rosacea. Subjects with rosacea applied the test moisturizer to their face and to one forearm twice daily for 4 weeks. The other forearm remained untreated as a control. Barrier function on the forearms was assessed instrumentally and using a dimethyl sulfoxide (DMSO) chemical probe. Stratum corneum hydration also was measured instrumentally. The dermatologist investigator evaluated each subject's rosacea condition over the course of the study, and subjects self-assessed their facial skin condition at study end. Instruments provided objective measures of stratum corneum barrier function and hydration on the face. [source]

Neuropathic Rosacea Becomes Subtype 5?
Dr. Nase reports that Neuropathic Rosacea may become Subtype 5 in the 'official' classification of rosacea soon at this post. Dr. Nase has his own classification of rosacea in his book and the NRS has developed the 'official' classification of four subtypes. Traditionally there has been only three stages classified. Classifying rosacea is no doubt a good idea since there is so much confusion on rosacea and having a standardization of classifications brings everyone into unity.

Bacteria with Mites Causes Rosacea?
Researchers have successfully demonstrated a possible role for bacteria associated with microscopic mites in the development of subtype 2 (papulopustular) rosacea, which is characterized by persistent redness with bumps (papules) and pimples (pustules). Many researchers have postulated that rosacea is a vascular disorder but mounting evidence suggests otherwise. The most significant statement of the research says,
"This indicates that the Bacillus bacteria found in the Demodex mite produce an antigen that could be responsible for the tissue inflammation associated with papulopustular rosacea,"

Antioxidant System Defect Hypothesis in Rosacea?
Summary
A possible link between superoxide dismutase activity and malondialdehyde level with the clinical manifestations of rosacea was investigated. We found differences in superoxide dismutase activities between mild rosacea (stages I and II) and severe involvement (stage III) groups, as well as between disease and control groups that were statistically significant (P < 0.05). In the mild involvement group (stages I and II), the superoxide dismutase activity was higher than in the control group (P < 0.05), while the malondialdehyde levels did not differ from the control.
In the severe involvement group (stage III), the superoxide dismutase activity was lower than in the control group (P < 0.05), and this was coupled to a raised level of malondialdehyde (P < 0.05). These findings clearly show that in the mild involvement phase of rosacea patients, superoxide dismutase activity was stimulated to protect the skin against reactive oxygen species so that the malondialdehyde levels were maintained. In contrast, in more severe disease, due to a decrease in the capacity of the antioxidant defence system, the malondialdehyde levels were increased. These findings support the 'antioxidant system defect hypothesis' in rosacea patients.

Possible Link Between Breast Cancer and Long-term Antibiotic Use
Many are naturally concerned what the possible long-term risks are for using antibiotics for the treatment of rosacea. A recent study indicates that there is a possible link between breast cancer and long-term antibiotic use. Who knows what other risks are associated with the long-term use of antibiotic treatment? If you keep taking antibiotics for years you will find out what they are for you at any rate. Dr. Sy in a post at the R-K yahoo group commented on this recent study and quotes the Academy of Dermatology statement on the recent findings from this study and I quote the following paragraphs:
"...While the findings of this research indicate there may be an association between antibiotic use and breast cancer, reviewers of the study state that it provides many more questions than it does answers. The research indicated that women who took antibiotics for more than 500 days or who had more than 25 prescriptions in the course of a 17-year period more than doubled their risk if breast cancer compared with women who had not taken any antibiotics....
...According to the Centers for Disease Control and Prevention, the overuse of antibiotics by the public has dramatically increased in the past 10 years. Please use this opportunity to educate your patients about the proper use of these drugs in order to ensure their safety..."

Propionibacterium Acnes Possible Factor in Rosacea
Based on the theory that rosacea shares the same inflammatory features of acne, a recent study showed that, just as the combination of benzoyl peroxide 1 percent and clindamycin 5 percent gel is a powerful treatment modality for reducing Propionibacterium acnes levels, it also significantly reduces the papules and pustules of rosacea, according to Debra L. Breneman, M.D. ...."Benzaclin, once daily, was found to be well tolerated and effective in the reduction of papules and pustules in patients with rosacea," said Dr. Breneman. Three Stages - Plewig and Kligman Classification of Rosacea
(Taken from their book, Acne and Rosacea, Second Edition, 1993)
Stage I: The erythema (redness) may persist for hours and days, hence the old term erthema congestivum (redness congestion). Erythema lasting only a few minutes is not early rosacea. Telangiectases becomes progressively prominent, forming sprays on the nose, nasolabial folds, checks, and glabella. Most of these patients complain of sensitive skin that stings, burns, and itches after application of a variety of cosmetics, especially certain fragrances and sunscreens. Trauma from abrasives and peeling agents readily induces long-lasting erythema, thus the facial skin is unusually vulnerable to chemical and physical stimuli.
Stage II: Inflammatory papules and pustules crop up and persist for weeks. Some papules show a small pustule at the apex, justifying the term papulopustular. The lesions are always follicular in origin, mainly in sebaceous follicles but also in the smaller and more numerous vellus follicles. Comedones do not occur. The deeper inflammatory lesions may heal with scarring, but scars are inconspicuous and tend to be shallow. Facial pores become larger and prominent. If there has been much solar exposure over decades, the stigmata of photodamaged skin becomes superimposed, namely yellowed, leathered skin (elastosis), wrinkles and solar comedones. The papulopustular attacks become more and more frequent. Finally, rosacea may extend over the entire face and even spread to the scalp, especially if the patient is balding. Itchy follicular pustules of the scalp are typical. Eventually, the sides of the neck as well as the retroauricular and presternal area may be affected.
Stage III: A small proportion of patients go on to develop more serious expressions of the disease, namely large inflammatory nodules, furunculoid infiltrations, and tissue hyperplasia. These derangements occur particularly on the cheeks and nose, less often on the chin, forehead, or ears. The facial contours gradually become coarse, thickened, and irregular. Curiously, patients may not notice these disfigurements. The deranged appearance becomes evident when photographs from previous years are reviewed. Finally, the patient shows diffusely inflamed, thickened, edematous skin with large pores, resembling the peel of an orange. These coarse features are due to extensively inflammatory infiltration, connective tissue hypertrophy, massive fibrosis and elastosis, diffuse sebaceous gland hyperplasia, and extreme enlargement of individual sebaceous glands forming dozens of yellowish unbilicated papules on the cheeks, forehead, temples, and nose. Thickened folds and ridges may create a grotesque appearance mimicking leonine facies of leprosy or leukemia. The ultimate deformity is the phymas, of which rhinophyma is the prototype.
